For fully insured group plans or off-exchange individual plans
Form 1095-B is a health insurance tax form required under the Affordable Care Act. This form reports the time period a member and their covered dependents had minimum essential coverage during the prior tax year.
Members who are part of a fully insured group plan or with an off-exchange individual plan use this form to file their taxes.
What we’ll be doing
Premera Blue Cross Blue Shield of Alaska will provide Form 1095-B to our members, as required.
This form will be distributed to members in January 2020 either through the mail or electronically. Members who sign up for electronic delivery will receive an email notification to retrieve the form from their online member account once it’s available. Those who sign up for electronic delivery will not receive a paper form in the mail.
This information will be communicated to employers later this month.

Electronic delivery is fast and easy! Make tax season easier for your clients by encouraging them to sign up and receive their Form 1095-B online. Signups must be submitted by December 31, 2019, to take advantage of this opportunity.

Make tax season easier! Premera Blue Cross Blue Shield of Alaska members can request an electronic 1095-B tax form from Premera for their 2019 health plan before December 31, 2019.
The 1095-B form shows the time period a Premera member and their covered dependents had minimum essential coverage during the prior tax year.
Here’s how to request to receive the 1095-B form electronically:
- Log in to your Premera member account. (Don't have an account? Create one today.)
- Under Manage My Account in the navigation, choose Tax Form 1095.
- Be sure to make your request by December 31.
Premera will send an email in January 2020 to members signed up to receive the electronic tax form once it is available. The subject line will say: IMPORTANT TAX RETURN DOCUMENT AVAILABLE.
You can also point your group clients directly to ready-to-share 1095-B messages and flyers. Log in to the secure employer website and select the link to the Employee Communications Center. Then select Understanding costs in the left-hand column, then Form 1095-B.
