To check eligibility
If your group client wants to know if an employee is eligible to be added to their Premera Blue Cross Blue Shield of Alaska health plan, the following information is required for ALL inquiries to access the account.
- First and last name of the member they are inquiring about
- Member’s ID number or last 4 digits of their Social Security number
- Member’s date of birth
To check claims or prior authorization status for non-sensitive claims
The information above, plus:
- Name of the provider
- Type of service
- Date of service or request
You would need to submit an authorization of release for the following reasons:
- If you are not able to check claims or prior authorization with this information
- If the inquiry is of a sensitive nature, the authorization would need to be filled out and any category the member is giving access to needs to be checked. If only the general claims box is checked then there will still be no access to sensitive info.
