Starting January 1, 2020, Premera Blue Cross Blue Shield of Alaska (Premera) is offering a new network for our Individual market health plans called Legacy and Dental Select. This network includes both medical and dental providers and all providers’ contracts have been updated as appropriate to support participation in this network.
Below are images of the new member ID cards for the Individual market that will help you identify patients that will use this network.
Individual member ID card:
- Network is Legacy and Dental Select
- Individual Plan is marked in the bottom right quarter of the card
- Express Scripts is the pharmacy benefit manager
- There’s a new customer service number and claims mailing address on the back of the card
BlueCard providers outside of the service area of AK and WA and considered out of network for non-emergent services and members will have out-of-network cost shares. Emergency services are always paid at the in-network cost share.
