We added a new Individual Plan Tools and Resources page so you can find everything you need to serve Premera individual plan members including:
- Direct sign-in to the individual secure web page and tools
- Prior authorization and appeals forms
- ID card information
- Customer service and care management contact information
- Alerts and updates about our individual plans
- Training guides and resources on how to use the online tools specific to our individual plans including claims submission, prior authorization submission, member eligibility and benefits lookup, and more
- Medical policies, code list, and InterQual criteria
- Care management programs
- Electronic Funds Transfer (EFT) and Electronic Remittance Advice (ERA) information
Our goal is to make it easy for you to find information and resources you need about our individual plans to better serve your patients and our members. To access the new page, select 2020 Individual Plans > Individual Plan Tools in the left-hand navigation bar on the provider homepage.
