Evidence shows that individuals are more likely to complete important screenings and maintain regularly scheduled appointments if they have a primary care provider. Select members will receive an email or a letter reminding them to schedule a preventive examination, including resources to select and connect with a provider. Select members include those who:
- Have chronic or complex conditions and are due for a preventive examination
- Are due for 2 or more of the following screenings and are due for a preventive examination:
- Colorectal cancer screening
- Breast cancer screening
- Cervical cancer screening
High Blood Pressure Informational Mailing
Federal Employee Plan (FEP) members with a diagnosis of hypertension will soon receive a mailing with information about how to take care of their hearts, including information about a free blood pressure monitor, the importance of regular visits, taking medication as prescribed, and making healthy lifestyle changes to improve blood pressure readings. HbA1c Testing Care Gap Letter
Individuals with a diabetes diagnosis should have their blood hemoglobin bA1c levels tested at least once every 12 months. Select members who haven’t yet received a test this year will receive a letter reminding them to schedule an appointment with their doctor to complete the test or discuss other options based on the member's diabetes care plan. Well-Child Informational Mailings and Growth Chart
The American Academy of Pediatrics (AAP) recommends that babies have at least 6 check-ups with their primary care provider by the age of 15 months. The AAP also recommends that children aged 2 and older see their doctor annually. View the tip sheet for information about patient claims and tips for success.
The parents/guardians of eligible members will receive the following:
- Growth chart for ages 1 to 3 months old
- Informational mailer 4 months to 11 years old
All mailings have relevant, age-appropriate information including:
- Recommended vaccine schedule
- Key milestones
- Anticipatory guidance
- How to find a primary care provider
Emergency Department Utilization Informational Mailer
This mailing provides members who have visited the emergency department 2 or more times in the past year with information about lower cost, and sometimes more accessible, alternatives.
