Premera Blue Cross is committed to ensuring access to high-quality healthcare through an extensive network of physicians, hospitals and other medical professionals.
That commitment to quality begins with high-quality providers. To ensure that our physicians and other practitioners are fully qualified, we start with certain basic selection criteria - including a valid license, training appropriate to the profession and acceptable malpractice claims history. We couple that with a rigorous and highly rated credentialing process that verifies your education, employment history and so on.

Our selection criteria
- Criteria for joining the Premera Blue Cross Physician and Provider Network includes:
- Current valid licensure or other authorization to practice
- Training appropriate to the profession
- Admitting privileges in good standing at a participating hospital, if applicable
- DEA certificate, if applicable
- Acceptable malpractice claims history
- Acceptable record with applicable disciplinary boards or other regulatory authorities
The decision whether to accept an applicant is based not only on the information gathered during the application process, but on network requirements as well.
Our credentialing process
Our credentialing process ensures that the doctors and other practitioners who provide care are fully qualified. We verify each practitioner's education, training and experience. We also check your background, employment history and malpractice history. We use credentialing criteria and standards to determine which healthcare practitioners we select.
The Premera Blue Cross credentialing program has won high ratings from the National Committee for Quality Assurance (NCQA) and has been continuously accredited since 1996. In addition, the national Blue Cross Blue Shield Association has published the program as an example of a successful credentialing program.
Under all our plans, physicians and other providers applying for a contract cannot see members under that contract until we credential you. Important: A physician's or provider's malpractice insurance needs to be effective before credentialing can be complete. We will notify you by sending a counter-signed contract and assigning an start date.
How to join
If you're interested in becoming a Premera healthcare provider, you can request an individual contract or a contract as an affiliate with one of our contracted IPAs/Medical Groups. In either case, you must also meet our credentialing and other criteria.

Send the completed form to along with copies of the following documents:
- DEA certificate
- All current state licenses
- Professional Liability Insurance Certificate
- Request for Taxpayer Identification Number and Certification
- Any explanations as required
If the information submitted in the application is complete, the process typically takes 60 to 90 days. We will assign the start date for your participation when we have completed the application and credentialing process.
