Health Connect Navigator
The Health Connect Plan offers a dedicated service center called the Health Connect Navigator. Call (800) 676-1411 and select Health Connect Navigator for help with any of the following:
- Choosing a PCP
- Finding a specialist
- Locating a clinic, urgent care center, or hospital
- Scheduling appointments
Representatives are available to help you Monday through Friday, 8:00 AM–5:00 PM Pacific Time.
24-Hour Nurse Line: Health information 24/7
When you have a quick question about a health issue, are looking for health information, or need to know where to get immediate care, nurses are available 24/7 to answer your questions and address your concerns. To speak to a consulting nurse,
