Your Employees Will See Two Provider Network Names on Their Plan Materials and ID Card

    For small groups that start or renew January 1, 2020 or later

    The provider network names on member ID cards and plan materials sent to your employees will now include the name of the Premera Blue Cross dental network associated with the following:

    • The embedded pediatric dental benefit required by the Affordable Care Act (ACA)
    • The family dental benefit, if applicable

    The Premera small group medical plan network names you can expect to see are:

    • Balance plans – Heritage Signature and Dental Choice
    • Choice plans – Heritage and Dental Choice
    • Peak Care plans – Tahoma and Dental Choice

    The change in network naming is in response to a directive of the Washington Office of the Insurance Commissioner. If you have questions about the network names or member ID cards, feel free to reach out to your Premera representative or producer.
