    Employee pays 100% until deductible is met, then pays a percentage of the cost (coinsurance)

    Premera Pathfinder plans (EPO)

    Save up to 10% over a traditional Premera PPO plan

    Office visit
    Employees have a $0 copay when seeing their designated PCP. $35-65 copay for non-designated PCP or specialist

    Provider network
    Heritage network with more than 42,000+ providers statewide. No out-of-network coverage for primary or specialist services

    Blue High Performance Network plans (HPN)

    Cost and coverage
    No out-of-network coverage for primary or specialist services, but emergency and urgent care is always covered

    Office visit
    Copay of $0-$35

    Provider network
    Heritage Prime network with 42,000+ providers statewide 

    2. Choose a pharmacy plan

    Add one of our 11 pharmacy plans to any of the medical plans except for the HSA PPO, which already includes pharmacy benefits.

    Essentials pharmacy plans

    Cost and coverage
    Keeps costs as low as possible by focusing on high-value drugs that are approved by the U.S. Food and Drug Administration (FDA)

    Office visit
    Employee pays a copay or percentage of drug cost

    Provider network
    Covers 30-day supply for retail and 90-day supply for mail order

    Orthodontia coverage
    The Optima with Ortho plan features orthodontia coverage for all ages

    Waiting period
    The Optima Voluntary plan requires a 12-month waiting period for major dental services
