    Healthy Rewards is Back.

    Your clients can earn rewards for improving their health.

    The Healthy Rewards program is back! We’re giving your Medicare Advantage clients incentives to help close gaps in their care. Getting the recommended screenings and scheduling important visits with their healthcare provider is key to keeping your clients’ healthy. We mailed information about the program to our Medicare Advantage members in mid-August.

    Here’s additional information if your clients have questions about the program.

    Healthy Rewards program

    • Members earn incentives when they attest to completed healthy activities based on their personalized eligibility. Members can earn up to $310 (based on eligible health conditions) in electronic or physical gift cards to: Home Depot, Shell, Regal, Kroger, Subway, Starbucks, Exxon, AMC, Safeway, or Panera.
    • Members can visit premera.com/healthyrewards for more information and easy access to the rewards program.
    • Members must complete the healthy activities by December 31, 2021.
    • Members can attest to completing the activity online or by phone by January 15, 2022, or by mail if postmarked by January 15, 2022.

    Care gap letters

    Members also received personalized care gap information in late August from Premera.

    • Members with care gaps received a letter and a list of recommended routine care they’ve completed or should discuss with their primary care provider.
    • Members who have care gaps in breast or colorectal cancer screening, hypertension, diabetes, and diabetic eye exams, also received informational brochures about these health conditions.
    • We emailed members that this information was coming if we had an email address for them.
