Reimbursement for Out-of-Network Provider Services

    You may have clients who receive services from out-of-network dentists who don’t bill Premera Blue Cross directly, or maybe they go to Costco to get their glasses. If the provider doesn’t bill Premera directly, they’ll need to fill out and submit a claim form and documentation to receive reimbursement for their out-of-pocket expense.

    Here are a few tips to share with your clients to help make the process go more smoothly.

    Use the correct form and fill it out completely. There are two different reimbursement forms available at medicareadvantage.premera.com:

    • Medical and vision claim reimbursement form
    • Dental claim reimbursement form
    • Bring the correct form with you to your visit. This will help you and the provider know at the time of your service what you’ll need to submit the claim.
    • Submit the completed form and requested documentation to the address listed on the form. Sending the claim to the wrong address will result in rerouting your paperwork.  This will slow down the process and your client’s repayment.
