The Premera HMO Core Plus plan requires that referrals be submitted directly to Premera HMO. Any other referral method can delay claims processing. There are two ways to place a referral for an HMO member:
- Use the Availity online tool (preferred method)
Providers can submit the referral to the specialist, see which provider is in-network, check referral status and more.
- Sign in to Availity to complete a request.
- In the Availity menu bar, select Patient Registration | Authorizations & Referrals.
- On the Authorizations and Referrals page, select Referral Request.
- Select Premera Blue Cross HMO as the payer to complete the steps for a request.
- Fax the Premera HMO referral form to 888-704-2091.
- This may delay processing; we will input this request into Availity for final processing.
- Use Availity to check status and get more information for the referral request.
Note: In February, we had a referral request coding issue in Availity which is now resolved. For the fastest turnaround time, submit your HMO referral requests directly through Availity. If you encounter any errors, call Availity Client Services at 800-282-4548, Monday-Friday, 8 a.m. to 8 p.m., ET.
