Premera HMO Core Plus Plan PCP Member Roster

    A key part of the Premera HMO Core Plus plan is a member’s primary care provider (PCP).
    At the beginning of a plan, Premera Blue Cross HMO assigns the member a PCP to ensure continuity of care and ease of referrals.

    PCP assignments are based on several factors, including a member’s previous claims history, location, age, and other factors. Members have the choice to change their PCP at any time by logging into their member account or calling customer service.

    We recommend checking your online PCP roster list monthly in Availity to determine if membership changes have occurred. Note: The PCP roster tool provides real-time membership changes.

    To view a list of your Premera HMO patients, sign in to Availity, select Payer Spaces and then select the Premera Blue Cross HMO logo. Click on the Resources tab and scroll down to the primary care provider (PCP) roster link to access the tool.

      • Review eligibility and benefits information
      • Place a referral, view referral updates, and see which provider is within the Sherwood HMO network
      • Submit prior authorizations and check status
