We are pleased to announce that The PolyClinic will be available to HMO members starting January 1, 2024. Members can choose a primary care provider (PCP) at The PolyClinic or be referred there for specialty care services if needed.
Please note: At this time, The PolyClinic’s obstetrics and gynecology providers will be out of network for Premera Blue Cross HMO Members. However, HMO members can find and select from several obstetrics and gynecology providers within the network, by signing into their account on Premera.com and utilizing the Find Care tool. Should a member have any questions, please direct them to our HMO Plus customer service team using the number listed on the back of their identification card.
