Premera is partnering with Home Access Health, a third-party vendor, to offer in-home test kits to select Medicare Advantage members to complete recommended preventive services in the privacy of their home.
In August, select members will receive a letter from Premera about preventive services along with the test kits. Members can choose to opt out of receiving the kits.
Members may receive the following kits based on their individual open care gaps:
- FOBT kit for colorectal cancer screening
- HbA1c test kit for diabetic blood sugar control
- Microalbumin urine test kit for diabetic kidney disease screening
Premera may ask you to review the list of members with open care gaps and ask for feedback on members we should add or exclude from program.
Once testing begins, primary care providers will receive two types of reporting:
- Information about care provided and any test results will be faxed or mailed directly to the member’s primary care provider; we recommend this information be included in your patient’s medical record.
- For high-volume providers, we will deliver monthly Excel workbooks with data pertaining to all clinical initiatives, including outreach results, care provided, and test results.
Members will receive their results by mail. Members with abnormal or positive results will also receive a phone call from the vendor.
