Premera receives hundreds of overpayment checks per month from providers. These checks typically need more patient or claim information so that we can correctly process the funds.
If you need to send us a check, please include a completed Medicare Advantage provider offset request form with the check that you return to us. The form gives us the details we need to make sure the check is processed correctly. It also saves a significant amount of administrative time for both provider offices and Premera if we have the information upfront.
