August Care Gap Member Outreach

  • August 4, 2022

Eligible members in Washington and Alaska received communications about clinical gaps in care, preventive care opportunities, or information related to chronic conditions. Here’s more information about each notification.

Home test kits for HbA1c and colorectal cancer screening

Eligible members in Alaska and Washington will be sent home test kits on August 22. This annual program aims to meet our members where they are and make it easier for them to receive these important screenings.

Based on eligibility, members will receive either a FIT (fecal immunochemical test) kit and/or a diabetic HbA1c test kit. There is no cost to members who receive a kit through this program.

Members will receive up to two reminder phone calls and one reminder letter to complete their test kit.

    High blood pressure informational mailing

Federal Employee Plan members with a diagnosis of hypertension will receive a mailing with information about how to take care of their hearts, including how to get a free blood pressure monitor, the importance of regular visits, taking medication as prescribed and making healthy lifestyle changes to improve blood pressure readings. View the tip sheet  for details.
