Outreach for Closing Member Care Gaps

    In July 2023, members with specific needs will receive:

    1. When a member is only due for one of the following cancer screenings, a letter about the importance of the cancer screening is sent.

      2. OR

    2. If a member is due for more than 1 cancer screening or chronic condition testing, a scorecard is prepared and sent for:

    The scorecard has personalized messaging. If members have not received a screening or test, more information about it will be printed, likewise, if members have received a screening or test, a quick “congratulations” message will be printed.
