When you refer patients for diabetic retinopathy screening, it can be challenging to ensure that they follow through with appointments. Recognizing this challenge, we're sharing a customizable referral template (shown below) with providers. When your patients are due for retinal eye exams, you can provide a referral form during a face-to-face visit, or mail it to your patient.
Eye care professionals tell us that patients are often confused about which exam is required. Delivering a written order to the eye care provider helps to insure that retinal screening is performed, above and beyond any general vision screening. The form also reminds the eye care provider to send results of the exam back to the PCP.
