Eligible members in Washington and Alaska received communications in
September about clinical gaps in care, preventive care opportunities, or
information related to chronic conditions.
Home Test Kits for HbA1c and Colorectal Cancer Screening
Eligible members in Alaska and Washington were sent
home test kits on or around September 20. This
annual program aims to meet our members where they are and make it easier for
them to receive these important screenings. There is no
cost to members.
Based on eligibility, members were sent either a
FIT (fecal immunochemical test) kit and/or a diabetic HbA1c test kit.
- Individuals aged 50-75
should be screened for colorectal cancer at regular intervals, for early
detection of abnormalities.
- Individuals
aged 18-75 with a diagnosis of type 2 diabetes, should receive a Hemoglobin A1c
test once every 12 months.
Members
will receive up to two reminder phone calls or one reminder letter to complete
their test kit.
Well-Child Informational Mailer or Well-Child
Growth Chart
The American Academy of Pediatrics (AAP) recommends
that babies have at least 6 checkups with their primary care provider by the
age of 15 months.
The parents/guardians of Federal Employee Plan
(FEP) members who are 1-3 months old will receive a growth chart and
parents/guardians of FEP members who are 4-15 months old will receive an
informational mailer. The growth chart and the informational mailer both have
the well child and vaccine schedules recommended by the AAP and the CDC for
children from birth to 15 months, key milestones, as well as topics discussed
at each well-child visit.
High Blood Pressure Informational Mailing
Federal Employee Plan (FEP) members with a
diagnosis of hypertension will receive a mailer about taking care of their
hearts, including information about a free blood pressure monitor, the
importance of regular visits, taking medication as prescribed and making
healthy lifestyle changes to improve blood pressure readings.
Care Gap Letters or Member Scorecard
FEP members will receive either:
- A letter about
the cancer screening they're due for, if they are due for only one of the
following screenings:
OR
- A scorecard if
they're due for 2 or more of the following screenings/tests:
- Diabetic HbA1c testing
- Breast cancer screening
- Cervical cancer screening
- Colorectal cancer screening
The scorecard has
personalized messaging. If members have not received a screening or test, more information will be noted. Likewise, if
members have received a
screening or test, a quick "congratulations" message will be noted.
Asthma Informational Mailing
FEP members with a diagnosis of persistent asthma
who have been prescribed medication for their condition will receive an
informational mailer about how to control their asthma. Members will be
encouraged to "take medications as prescribed," among other tips
including avoiding triggers, getting a flu shot, scheduling regular visits with
a provider, exercising safely, and developing an asthma action plan.
Diabetes Informational Mailer
Members with a diagnosis of diabetes, should
receive the following once a year:
- Hemoglobin A1C test
- Cholesterol and triglycerides test
- Retinal eye exam
- Urine test
The purpose of this initiative is to
educate and remind our members of the importance of following their diabetes
care plan, regular exercise, and important tests. The call to action is to
schedule an appointment with their doctor to complete the test or discuss other
options based on the member's diabetes care plan.
