Premera and Home Access Health (Home Access) will be sending in-home health screening kits to members who are due for select tests. Home Access will order and send hemoglobin A1c (HbA1c) test kits to members with diabetic HbA1c testing gaps, microalbumin
urine (MAU) test kits to member with diabetic nephropathy (kidney disease) testing gaps, and fecal immunochemical test (FIT) kits to members with colorectal cancer screening gaps. The program will run between September and December 2021.
Members participating in these programs receive their results in two to three weeks of the lab's receipt of the member's sample(s) and providers will receive copies of the results. We suggest adding these screening results to your patient's medical record
and population health system.
Members and providers will also receive a call if the results are positive/abnormal. If your patient's results are positive/abnormal, discuss the findings and ensure follow-up care takes place.
HbA1c testing for members with diabetes
Individuals ages 18-75 with a diagnosis of diabetes should receive an HbA1c test once every 12 months.

MAU for nephropathy testing for members with diabetes
Individuals ages 18-75 with a diagnosis of diabetes should have a nephropathy screening test once every 12 months, unless receiving medical attention for nephropathy.

FIT for colorectal cancer screening
Individuals ages 50-75 years are screened for colorectal cancer at regular intervals for detection of abnormalities.
