The Washington State Health Care Authority is reaching out to primary care providers to help shape implementation of the Washington Primary Care Transformation Model.
The Washington State Health Care Authority (HCA) is working with the state’s purchasers, providers, and others to improve
primary care for Washingtonians. The overarching goal of these transformation efforts is to achieve the quadruple aim of enhancing patient experience, improving population health (i.e., health outcomes), reducing costs, and improving the work life
of health care providers, including clinicians and staff.
For the Primary Care Transformation Model to work, the HCA needs to understand a provider's current capacities and needs to implement the envisioned
model. Your perspective is critical as the HCA works to design a sustainable primary care model for patients, providers, payers, and purchasers.
Survey
The HCA survey is designed for completion by an administrative leader, with consultation where necessary from clinicians. Please provide responses at an organizational
level (rather than individual clinicians) and only one response for your clinic/practice. It should take about 20-25 minutes.
