    You are eligible to participate in the Health Connect Plan if you live in King or Snohomish counties. It’s important that eligible employees and their family members reside near the Health Connect Plan facilities and providers, which are located primarily on the Eastside.

    The plan includes two networks:

    • The Health Connect Network
      • Eastside Health Network
      • Allegro Pediatrics
      • Living Well Health Center
      • Kinwell Health
      • Seattle Children’s hospital and Children’s University Medical Group (CUMG)
        • Pediatric specialty services provided at Seattle Children’s hospital by any of the 600+ pediatric specialists that make up CUMG are available to Health Connect members at the Health Connect Network benefit level.
        • Note: Seattle Children’s primary care physician services still apply to the Extended Network benefit level.
      • Extended network consists of the same broad, nationwide network of Premera Blue Cross providers that is available under the Health Savings plan. The network includes Virginia Mason, UW Medicine, Swedish Health Service, and Providence Health & Services. If you see providers in the Extended network or out-of-network, you will be subject to a deductible as well as copays and coinsurance, so your out-of-pocket costs may be higher.
